Before beginning any Life Assessment Systems service — including LifeTest, PEMF Therapy, Qi Coil, or Avazzia microcurrent sessions — we ask all clients to review and acknowledge the following informed consent. Your understanding and agreement help us ensure safe, respectful, and transparent care.

Signing this form indicates that you are voluntarily and with full knowledge willing to undergo one of the following procedures – LifeTest, PEMF, Qi Coil and/or Avazzia micro current.

At no time will the technician state, suggest, or imply that they, Life Assessment Systems, LifeTest, or any other procedure can diagnose or treat any types of ailments or diseases whatsoever. Only medical doctors can diagnose symptoms of ailments and disease, and prescribe medication for such.

LifeTest, also known as EAV, is a Class 2 medical device and is considered investigative only and not diagnostic.

At no time will there be any implied or stated indication to any client to discontinue or alter any care that they are receiving under the direction of a physician.

This procedure is not intended, implied, or stated to take the place of any conventional medical test or diagnostic procedure.

I have fully read and understand the above information, and I hereby give consent to the LifeTest, PEMF, Qi Coil or Avazzia procedure.
